We accept most major insurances and accept VISA & MasterCard. We will gladly bill your insurance company for you, but please be aware that patients are responsible for any deductibles, co-pays, and non-covered, or non-authorized services. In the state of Washington, clients are allowed to see a physical therapist without a doctor’s prescription, however most insurance companies require one. We therefore recommend that you contact your insurance provider prior to your first visit to determine if any pre-authorization or referral is required.
For your first visitThe initial evaluation will be 45-60 minutes and will include an interview, physical examination, and treatment as time allows. If you you are unable to fill out your paperwork ahead of time (New Patient Form PDF | DOC), then please arrive 10 minutes prior to your scheduled appointment to do this. Wear comfortable clothing that will allow easy access to the body part being evaluated. If you are being evaluated for orthotics, please bring a few shoes that you regularly wear and any past orthotic devices you have had. You will need to wear shorts or pants that can be rolled above the knee.
Remember to bring
- Your insurance card, claim number, and/or billing information.
- The prescription for physical therapy from your doctor.
